Degree/Certificate Change Request


If you wish to change the degree or certificate program declared on your record, please complete this form. Make sure to speak with your Pathways Advisor, Financial Aid Advisor, and/or Military & Veteran Success Coach prior to requesting a change to your major as:

  • It may have financial aid or other funding implications;
  • It may change the number of courses you are required to take to complete your degree or certificate program;
  • Transfer credit may need to be re-evaluated, requiring a new Transfer Credit Evaluation Request form to be completed.

Requests to change your declared degree/certificate program can only take effect in current and future semesters, when your student record is active. The deadlines for degree/certificate program changes are as follows:

  • Spring 2024:  January 31, 2024
  • Summer 2024: June 6, 2024
  • Fall 2024: September 4, 2024

Once a semester deadline has passed, your request for a program change will still be processed, just for the following semester.
